Do I need to tell my current provider?
No. Once your nomination is updated, future electronic prescriptions can be sent to Bradley Stoke Pharmacy automatically.
Will my prescription history move as well?
Your medication record remains part of your NHS care. We will receive future nominations rather than manually transferring paper history ourselves.
Can I nominate on behalf of someone else?
You should only complete the form for another person when you have the right authority to do so and their consent is clear.
What if I only need a temporary change?
Please speak with the team. We can advise on the best route if your move is short term or linked to a temporary address.
Does nomination cost anything?
No. Updating your nomination is free.
